Ringkasan KTISustainable Development Goals (SDGs) are the global frameworks by which one of the goals is specifically sets out to ensure healthy lives and promote well-being for people at all ages. Literally, Indonesia still has many challenges to reach such global targets since Indonesia is in the fight against various complex health problems, such as high maternal and mortality rates, stunting, diphtheria outbreaks and other related health issues. These growing diseases are like the iceberg phenomenon. The action to solve those health problems is still curative rather than preventive and promotive. The preventive and promotive action has not been taken and developed although they can be applied by screening through health risk factors and early detections. However, it is difficult to apply with the lack of health data. The healthcare institution and industry historically has generated large amounts of data driven by record keeping, compliance & regulatory requirements, and patient care. Nevertheless, most data are recorded in the form of paper-based database which can be potentially lost, less accurate or incomplete, and hard to update and backup. Therefore, electronic integrative health care data needs to be created with an overall vision to support health prevention and promotion in Indonesia. In an era of disruption technology nowadays, where all aspects of life cannot be separated from the digital system, integrated electronic health record can be set as a solution to the health issues in Indonesia. Currently, internet service users in Indonesia reached 143.26 million with a penetration of 54.68% of all people at all age (APJII, 2017). Thus, this condition becomes an opportunity for Indonesia to solve the existing health issues with digitalization system. In this paper, I propose new model of integrated health care in order to improve the quality health care in Indonesia. This model aims to integrate all health related data from all health institutions to encourage and determine better access, quality, user satisfaction and efficiency. Prior to the establishment of integrated electronic health records, clinicians and health records specialists were the central “integrators” of patient information. However, in this model, the massive quantities of integrated health data hold the promise of supporting a wide range of medical and healthcare functions, including clinical decision support, disease surveillance, and population health management. The model of integrated healthcare potentially can run three main functions such as predictive analysis, prescriptive analysis and genomic analysis. When big data are synthesized and analyzed, healthcare providers and other stakeholders can develop more thorough and effective diagnoses and treatments, treatments and interventions. In particular, this model may bring lower cost but with better outcomes, more accurate analytic to patient profiles, more predictive in determining large-scale disease profiling. In addition, some actions can also be set, such as applying, early intervention initiatives based on risk factors, and collecting and publishing data for medical purposes which in turn, assisting patients in determining the care protocols or regimens at the best value. The implementation of this model will significantly affect Indonesia Sehat program by decreasing health problems through curative, promotive and preventive medical services. Yet, it still requires holistic multidimensional collaborative approach from stakeholders, healthcare providers or agencies, and information technology experts.  In addition, there are three model implementation strategies including literacy of data, literacy of technology and literacy of human. The literacy of data by health workers leads to increasing ability to analyze data, make a conclusion based on data, and matching data. The literacy of technology is required for IT expert to provide an insight about how technology works and data security. Meanwhile, the literacy of human, deals with the ability to communicate and pay attention to human aspects such as commitment to legal ethics as well as empathize for patients in the era of digitalization. Finally, integrated health care model described in this paper will be regarded as a strategic solution to future healthcare system in Indonesia. By digitizing, combining and effectively using big data, significant benefits on healthcare can potentially be achieved. The data can be used to detect diseases at earlier stages so that patients can be treated more easily and effectively, manage individual and population health issues and also detect healthcare fraud more quickly and efficiently.
